Roles & Responsibilities

• Provide individualized physical therapy assessments and treatments in patients' homes
• Develop and implement comprehensive care plans to restore mobility and function
• Collaborate with physicians, nurses, and home health aides to ensure coordinated care
• Educate patients and caregivers on safe exercise programs and fall prevention
• Maintain accurate and timely documentation of patient progress
• Adapt therapy techniques to meet the unique needs of each home environment

Qualifications

• Valid Physical Therapist license in Iowa or compact privilege
• Current BLS certification
• Experience in home health or community-based care preferred
• Ability to travel between patient homes within the service area
• Experience with Epic EMR is a plus, but not required

Knowledge & Skills

• Strong clinical reasoning and assessment skills
• Knowledge of home health regulations and OASIS documentation
• Excellent communication and patient education abilities
• Self-motivated with strong time management skills
• Compassionate and patient-centered approach
