Roles & Responsibilities

• Assist physical therapists in implementing personalized treatment plans to help patients regain mobility and independence
• Guide residents through therapeutic exercises and activities tailored to their recovery goals
• Monitor patient progress and document responses to treatment accurately
• Educate patients and their families on home exercise programs and safety techniques
• Collaborate with nurses, therapists, and physicians to deliver comprehensive rehabilitative care
• Create a compassionate and encouraging environment that supports healing and dignity

Qualifications

• Active Physical Therapist Assistant license in New Hampshire
• Graduate of an accredited Physical Therapist Assistant program
• Experience in skilled nursing or rehabilitation settings preferred
• Strong communication and interpersonal skills
• Ability to work collaboratively in a fast-paced healthcare environment
• Current CPR certification

Education

• Associate degree in Physical Therapist Assistant from an accredited program

Knowledge & Skills

• Knowledge of therapeutic exercises, modalities, and rehabilitation techniques
• Understanding of Medicare, Medicaid, and insurance documentation requirements
• Proficiency with electronic medical records and documentation systems
• Compassion and empathy when working with older adults and patients in recovery
• Excellent time management and organizational skills
