Roles & Responsibilities

• Implement individualized physical therapy treatment plans developed by the supervising Physical Therapist to help patients regain mobility and independence
• Provide direct patient care including therapeutic exercises, gait training, and modalities in a skilled nursing and short-stay rehab setting
• Document patient progress accurately and communicate with the rehab team to adjust treatments as needed
• Educate patients and families on home exercise programs and safety strategies
• Contribute to a compassionate, supportive environment that promotes dignity and quality of life for residents

Qualifications

• Active Physical Therapist Assistant license in New Hampshire (or eligible to obtain)
• Graduation from an accredited Physical Therapist Assistant program
• Current CPR/BLS certification preferred
• Experience in skilled nursing, rehab, or long-term care settings is a plus
• Strong communication and teamwork skills

Knowledge & Skills

• Knowledge of rehabilitative techniques, therapeutic exercise, and modalities
• Ability to document care in electronic health records
• Compassion, patience, and a patient-centered approach
• Effective time management and organizational skills
