Physical Therapist Assistant - Skilled Nursing Facility
Location: Concord, NH
Start Date: 08-31-2026
Contract Type: Perm
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM; weekend rotation as needed
Hours per Week: 40
Bill Rate: $29-$34/hour

Roles & Responsibilities
- Implement individualized physical therapy treatment plans developed by the supervising Physical Therapist to help patients regain mobility and independence
- Provide direct patient care including therapeutic exercises, gait training, and modalities in a skilled nursing and short-stay rehab setting
- Document patient progress accurately and communicate with the rehab team to adjust treatments as needed
- Educate patients and families on home exercise programs and safety strategies
- Contribute to a compassionate, supportive environment that promotes dignity and quality of life for residents
Qualifications
- Active Physical Therapist Assistant license in New Hampshire (or eligible to obtain)
- Graduation from an accredited Physical Therapist Assistant program
- Current CPR/BLS certification preferred
- Experience in skilled nursing, rehab, or long-term care settings is a plus
- Strong communication and teamwork skills
Knowledge & Skills
- Knowledge of rehabilitative techniques, therapeutic exercise, and modalities
- Ability to document care in electronic health records
- Compassion, patience, and a patient-centered approach
- Effective time management and organizational skills
