Roles & Responsibilities

• Provide direct patient care in a home health setting, focusing on wound care including wound vacs and ostomies
• Manage post-surgical wounds and administer IV antibiotics with PICC line care
• Monitor and educate patients with cardiac conditions such as CHF and pneumonia
• Coordinate with interdisciplinary team to ensure comprehensive care
• Document all patient interactions and treatments using HomeCare HomeBase EHR

Qualifications

• Active LPN/LVN license in California
• BLS certification (AHA required)
• Valid driver's license and auto insurance
• Minimum 2 years of home health experience
• Experience with HomeCare HomeBase (HCHB) EHR
• Proven wound care skills including wound vacs and ostomies

Knowledge & Skills

• In-depth knowledge of wound care management and treatments
• Proficiency in HomeCare HomeBase electronic health records
• Understanding of cardiac conditions, heart failure, and respiratory care
• Ability to perform IV therapy and PICC line maintenance
• Strong patient education and communication skills
• Time management and ability to prioritize visits
