Roles & Responsibilities
- Provide hands-on nursing care for clinic patients within the scope of LPN licensure, ensuring high-quality, compassionate service.
- Take and record vital signs, perform focused patient assessments, and triage patient calls to support timely care.
- Manage medication refills and complete prior authorizations to keep patient treatments on track.
- Serve as a primary liaison between patients, providers, and clinic staff to coordinate care and follow-up.
- Provide direction and oversight to CNAs and Schedulers, supporting an efficient care team.
- Perform accurate clinical documentation to maintain clear, compliant patient records.
- Collaborate with providers, coordinators, clinical managers/supervisors, and RNs to meet patient needs and clinic goals.
Qualifications
- Graduate of an accredited nursing program (LPN)
- Current State LPN license (active and unencumbered)
- Basic Life Support (BLS) certification
- Experience taking vital signs, triaging patient calls, and managing medication refills
- Familiarity with prior authorization processes and clinical documentation
- Comfort supervising CNAs and coordinating with interdisciplinary teams
Education
- Graduate of a Nursing Program (LPN)

- Triage and telephone triage skills
- Medication refill processing
- Prior authorization knowledge and completion
- Clinical documentation and charting
- Basic Life Support (BLS)
