Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents
• Administer medications and treatments as ordered by physicians
• Monitor patient vital signs and promptly report changes to the care team
• Collaborate with registered nurses and other healthcare professionals to ensure quality care
• Assist residents with daily living activities, promoting independence and dignity
• Maintain accurate and timely documentation of patient care
• Educate patients and families on health management and care plans
• Ensure a safe, clean, and comforting environment for all residents

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania
• Current BLS/CPR certification
• Graduate of an accredited practical nursing program
• Compassionate, patient-centered approach to care
• Ability to work effectively as part of a multidisciplinary team
• Previous long-term care or skilled nursing experience is a plus

Knowledge & Skills

• Medication administration and documentation
• Wound care and basic nursing procedures
• Patient assessment and vital sign monitoring
• Electronic health records (EHR) charting
• Effective communication with patients, families, and team members
• Critical thinking and problem-solving in clinical situations
