Shift Type: Monday-Friday day shifts; some weekends as needed (day shift, specific times vary)

Roles & Responsibilities

Provide direct nursing care to clinic patients within the scope of LPN licensure, ensuring safety and compassion
Take and accurately document vital signs and patient assessments to inform provider decision-making
Triage patient calls, prioritize concerns, and escalate to providers or RN as appropriate
Manage prescription refill requests and complete prior authorizations in a timely manner
Serve as a primary liaison between patients, staff and providers to coordinate care and improve patient experience
Support and direct CNAs and schedulers in daily workflows to maintain smooth clinic operations
Collaborate with interdisciplinary teams including providers, coordinators and clinical managers to meet patient needs
Follow state and federal licensure requirements and participate in continuing education to maintain best practices

Qualifications

Current State LPN license (required)
Graduate of an accredited Nursing/LPN program
BLS (Basic Life Support) certification required
Comfortable triaging patient calls, managing vital signs, and handling prescription refills/prior authorizations
Ability to give direction to CNAs and schedulers and take direction from providers, RNs and clinical managers
Willingness to work occasional weekends as needed

Education

Graduate of an accredited Nursing Program (LPN/Vocational Nursing)

Knowledge & Skills

Patient assessment and taking vital signs
Triage skills for phone and clinic presentations
Experience with prescription refill workflows and prior authorization processes
Strong communication and patient liaison abilities
Team collaboration within interdisciplinary clinical settings
Basic life support and adherence to licensure continuing education standards
