Roles & Responsibilities

• Provide compassionate pastoral and spiritual care to patients and families in home, assisted living, and skilled nursing settings
• Conduct meaningful home visits across multiple points of care and travel safely between locations
• Offer bereavement support, counseling, and spiritual guidance tailored to individual needs
• Collaborate with the interdisciplinary hospice team (nursing, social work, volunteers) to coordinate holistic care
• Document encounters and care plans clearly and timely in patient records
• Support and train hospice volunteers and engage community resources to enhance patient support
• Participate in on-call rotations or team meetings as required to support continuity of care

Qualifications

• Ordained, commissioned, licensed, or endorsed by a recognized pastoral/spiritual community (required)
• Use of personal automobile with valid driver's license and required auto liability insurance; maintain vehicle in good working order
• Experience or training in pastoral/spiritual care (required)
• Comfort with home visits, skilled nursing, and assisted living environments
• Bereavement support and counseling experience preferred
• Master's degree in a related field preferred (but not required)

Education

• Bachelor's degree in a related field (required) OR
• Bachelor's in an unrelated field plus 2 years pastoral/spiritual or medical experience in an agency/institution OR
• Associate's degree in a related field plus 2 years experience as a chaplain
• Master's degree in a related field preferred

Knowledge & Skills

• Skilled in pastoral/spiritual care, counseling, and bereavement support
• Familiarity with hospice principles, pain and symptom management, and interdisciplinary care
• Strong interpersonal and communication skills with patients, families, and team members
• Ability to document care appropriately and maintain professional boundaries
• Cultural sensitivity and ability to support diverse spiritual beliefs and practices
• Reliable transportation and ability to travel to multiple points of care
