Roles & Responsibilities

• Provide compassionate direct patient care including assistance with activities of daily living
• Monitor and report changes in patient condition to nursing staff promptly
• Assist with safe transfers, ambulation and positioning to promote comfort and mobility
• Maintain a clean, organized and safe environment for residents and staff
• Collaborate with nurses and interdisciplinary team to support individualized care plans
• Document care and interventions accurately and timely in accordance with facility policy

Qualifications

• Current CNA certification (or state equivalent) required
• Valid work authorization for the United States
• Demonstrated ability to provide compassionate, patient-centered care
• Comfortable with lifting/transfers and physical demands of bedside care
• Willingness to complete facility onboarding, background checks and required immunizations
• Prior long-term care or skilled nursing experience is a plus

Education

• High school diploma or GED preferred
• Completion of an accredited CNA training program and active CNA certification

Knowledge & Skills

• Strong interpersonal and communication skills with patients and families
• Basic clinical skills: vital signs, ADL assistance, feeding and hygiene support
• Ability to follow care plans, safety protocols and infection control practices
• Dependable, punctual, and able to work collaboratively on a care team
• Attention to detail for accurate charting and reporting
