Roles & Responsibilities

• Provide specialized diabetes education and consultation to patients, families, and communities
• Assess and evaluate diabetes-related services to improve patient outcomes
• Collaborate with the primary care clinic team to deliver comprehensive care
• Apply strong critical thinking and triage skills to manage patient needs effectively
• Empower individuals to take control of their diabetes through evidence-based guidance
• Support the clinic in delivering high-quality, patient-centered care

Qualifications

• Active Registered Nurse (RN) license in good standing
• Current BLS/ACLS certification from the American Heart Association
• Certified Diabetes Educator (CDE) certification obtained within the last 3 years
• Minimum of 3 years recent experience in the specialty
• Clinic experience is mandatory
• Strong triage skills with knowledge of the Emergency Severity Index (ESI)
