Roles & Responsibilities

• Evaluate patients' physical therapy needs and develop individualized treatment plans
• Implement therapeutic exercises and modalities to improve mobility, strength, and function
• Collaborate with the interdisciplinary care team to ensure comprehensive patient care
• Document patient progress and adjust treatment plans as needed
• Educate patients and families on home exercise programs and fall prevention
• Provide compassionate, patient-centered care in a skilled nursing and rehabilitation setting

Qualifications

• Valid Physical Therapist license in Massachusetts
• New graduates are encouraged to apply
• Experience in skilled nursing or rehab settings is preferred but not required
• Current BLS/CPR certification
• Strong communication and interpersonal skills

Knowledge & Skills

• Knowledge of physical therapy assessment techniques and treatment modalities
• Ability to develop and modify care plans based on patient response
• Proficiency with electronic medical records (EMR) documentation
• Understanding of Medicare and insurance billing guidelines for rehab services
• Skill in patient education and family counseling
• Collaborative approach to interdisciplinary care
