Roles & Responsibilities

• Evaluate and treat patients with mobility, balance and functional limitations
• Develop and implement individualized physical therapy plans
• Collaborate with interdisciplinary teams to optimize patient outcomes
• Educate patients and families on exercises and injury prevention
• Document patient progress accurately and maintain compliance
• Empower patients to regain independence and improve quality of life

Qualifications

• Active Physical Therapist license in Delaware or compact eligibility
• Graduate from an accredited Physical Therapy program
• BLS certification required
• New graduates are welcome to apply
• Previous experience in skilled nursing or rehabilitation is a plus

Knowledge & Skills

• Strong clinical reasoning and patient assessment skills
• Excellent communication and interpersonal skills
• Proficiency with electronic health records (EHR)
• Ability to adapt treatment plans to diverse patient needs
• A compassionate, patient-centered approach to care
