Roles & Responsibilities

• Evaluate patients' physical therapy needs and develop individualized treatment plans.
• Implement therapeutic exercises and modalities to improve mobility, strength, and function.
• Collaborate with nurses, physicians, and other healthcare professionals to coordinate patient care.
• Educate patients and their families on treatment plans and home exercise programs.
• Maintain accurate patient records and documentation in compliance with facility policies.
• Participate in discharge planning and recommend adaptive equipment when needed.

Qualifications

• Current New Hampshire Physical Therapist license in good standing.
• Graduation from an accredited Physical Therapy program (DPT preferred).
• Previous experience in skilled nursing, rehabilitation, or long-term care settings is a plus.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassion and dedication to improving patients' quality of life.

Knowledge & Skills

• Strong knowledge of physical therapy techniques and evidence-based practices.
• Familiarity with Medicare, Medicaid, and insurance documentation requirements.
• Proficient in electronic medical records (EMR) systems.
• Ability to assess patient progress and adjust treatment plans accordingly.
• Understanding of safety and infection control protocols in a healthcare setting.
