Roles & Responsibilities

• Conduct comprehensive occupational therapy evaluations to understand each resident's functional needs and goals.
• Develop and implement personalized treatment plans that help residents regain independence in daily activities.
• Collaborate with the interdisciplinary care team to ensure holistic, patient-centered rehabilitation.
• Educate patients, families, and caregivers on adaptive techniques and home safety strategies.
• Document progress accurately and adjust treatment interventions as needed to maximize outcomes.
• Advocate for each resident's highest level of function and quality of life.

Qualifications

• Active Occupational Therapist (OT) license in Vermont or eligibility to obtain one before start.
• Graduate of an accredited Occupational Therapy program.
• Current BLS/CPR certification.
• Previous experience in skilled nursing, long-term care, or rehab setting preferred but not required.
• Must be comfortable with geriatric patient population.

• Knowledge of Medicare documentation and reimbursement guidelines
• Team collaboration and adaptability
