Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration, wound care, and treatments as ordered
• Monitor and document vital signs, patient conditions, and responses to interventions
• Collaborate with the care team to develop and implement individualized care plans
• Assist residents with daily living activities while promoting independence and dignity
• Communicate effectively with patients, families, and healthcare providers
• Maintain accurate and timely documentation in compliance with facility policies

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania
• Current BLS/CPR certification
• New graduates welcome; prior LPN experience in long-term care or skilled nursing is a plus
• Ability to pass background checks and comply with facility requirements
• Strong clinical assessment and critical thinking skills
