Roles & Responsibilities

• Provide compassionate direct nursing care to residents in accordance with individualized care plans
• Administer medications and treatments accurately and safely
• Monitor vital signs and promptly report any changes in resident condition
• Document patient care thoroughly and in a timely manner
• Collaborate with registered nurses, physicians, and other healthcare professionals
• Educate residents and their families on care plans and health management

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania
• Current BLS/CPR certification
• Previous experience in long-term care, skilled nursing, or similar setting preferred
• Strong clinical assessment and documentation skills
• Ability to work independently and as part of a collaborative team

Knowledge & Skills

• Solid knowledge of nursing principles, medication administration, and resident care
• Excellent communication and interpersonal skills
• Compassionate, patient-centered approach to care
• Proficiency with electronic health records and documentation
• Ability to prioritize tasks and respond calmly in a fast-paced environment
