Roles & Responsibilities

Provide compassionate direct patient care and assist with activities of daily living
Monitor and document patient conditions and report changes to nursing staff
Support safe mobility and transfers, following facility protocols
Assist with meals, hygiene, and comfort measures to promote dignity
Maintain a clean and safe environment in accordance with infection control practices
Collaborate with nurses and interdisciplinary team to deliver person-centered care
Contribute to accurate charting and timely communication with the care team

Qualifications

Current CNA certification (must be active and valid)
Certification or eligibility to be listed on the Pennsylvania CNA registry
Basic life support (BLS) or CPR certification preferred or required per facility
Ability to pass background check and meet facility onboarding requirements
Strong communication skills and a team-oriented attitude
Reliable, compassionate, and committed to patient-centered care

Education

High school diploma or equivalent preferred
Certified Nursing Assistant (CNA) certification required

Knowledge & Skills

Skilled in assisting with ADLs and personal care
Knowledge of infection control and safe patient handling
Ability to accurately document care and observations
Competent with basic patient safety and mobility techniques
Strong interpersonal skills and a focus on patient dignity
Familiarity with long-term care workflows is a plus
