Roles & Responsibilities

Provide compassionate direct care and assistance with activities of daily living
Assist residents with hygiene, dressing, mobility and feeding as needed
Monitor and record vital signs and report changes to the nursing team
Document care accurately and timely in the resident record
Support a safe, clean, and comfortable environment for residents
Communicate clearly and kindly with residents, families, and care team
Participate in team huddles and follow care plans to improve outcomes

Qualifications

Current CNA certification (or eligible) — Pennsylvania certification preferred
Current CPR/BLS certification (or willingness to obtain)
Up-to-date immunizations and TB screening per facility policy
Comfortable performing physical aspects of resident care and lifting safely
Reliable, compassionate, and team-oriented attitude

Education

High school diploma or GED preferred
Completion of an accredited CNA training program and current certification

Knowledge & Skills

Strong knowledge of ADLs and basic nursing care
Ability to take and record vital signs accurately
Infection control and hand hygiene practices
Basic charting and documentation skills
Excellent communication and interpersonal skills
Ability to observe, prioritize, and report changes in condition
