Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration, wound care, and vital sign monitoring
• Collaborate with registered nurses and the care team to develop and implement individualized care plans
• Accurately document patient assessments, interventions, and outcomes
• Educate patients and families on health management and discharge instructions
• Maintain a safe, clean, and therapeutic environment for residents and staff
• Respond promptly to patient needs and changes in condition, escalating as appropriate

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico or compact state
• Current BLS/CPR certification
• COVID-19 vaccination or approved medical/religious exemption
• State or federal photo ID to meet registry compliance requirements
• Prior LPN experience in long-term care or skilled nursing is a plus, but new graduates are welcome to apply

• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Proficient in electronic health record (EHR) documentation
• Ability to adapt to a fast-paced environment
• Knowledge of infection control practices and state regulations
