Roles & Responsibilities

• Deliver compassionate, hands-on nursing care to residents throughout the evening shift
• Administer medications and treatments accurately and on schedule
• Monitor and document patient conditions, reporting changes to the care team
• Collaborate with RNs, CNAs, and physicians to keep care plans on track
• Support residents and families with kindness, patience, and clear communication
• Maintain a safe, clean, and welcoming environment for everyone in the facility

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of New Hampshire
• Current BLS certification
• Prior experience in a skilled nursing or long-term care setting is a plus
• Completed COVID-19 vaccination series or a documented religious or medical exemption
• Ability to bring a state or federal issued ID and wear a photo badge at all times
• Strong communication skills and a genuine passion for patient care

• Solid clinical assessment and medication administration skills
• Comfortable with electronic medical records and documentation
• Excellent time management during a busy evening shift
• Team-first mindset with strong collaboration habits
