Shift Type: Monday-Friday, 8:00 AM - 4:00 PM or 9:00 AM - 5:00 PM; weekends as needed (day)
Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Provide skilled physical therapy evaluations and treatment plans in patients' homes
- Deliver patient-centered mobility, balance, and functional training to improve independence
- Document encounters accurately and timely using facility/charting systems (Epic preferred)
- Collaborate with interdisciplinary teams and families to coordinate safe discharge plans
- Educate patients and caregivers on home exercise programs and fall prevention
- Participate in required competency testing and maintain mandatory health screenings
Qualifications
- Current Physical Therapy license (license/certificate verification required)
- Basic Life Support (BLS) certification
- Home health experience and comfort working independently in patients' homes
- Epic experience preferred but not required
- Completion of Trinity Specialty Specific Competency Testing and Skills Checklist as required
- Up-to-date immunizations and successful completion of required health screenings (fit test, drug screen, TB testing, MMR, Varicella, Tdap, COVID-19, influenza)
Knowledge & Skills
- Strong home health assessment and clinical decision-making skills
- Excellent patient education and caregiver training capability
- Proficient documentation and charting practices; Epic familiarity is a plus
- Safe mobility, transfers, gait training, and fall-risk mitigation techniques
- Time management and routing skills to maximize daily caseload efficiency
- Professional communication with patients, families, and interdisciplinary teams
