Roles & Responsibilities

• Provide compassionate nursing care to residents in a skilled nursing and rehab facility
• Administer medications and treatments as prescribed by physicians
• Monitor patient vital signs and report changes to the healthcare team
• Document patient care accurately and timely in electronic medical records
• Collaborate with interdisciplinary team to ensure optimal patient outcomes
• Educate patients and families on health management and discharge plans

Qualifications

• Active Licensed Practical Nurse (LPN) license in Vermont or compact state
• Minimum 1 year of LPN experience in long-term care or rehab setting
• BLS certification from American Heart Association
• Ability to work 16-hour weekend shifts
• Travel experience preferred but not required

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Proficiency with EHR systems and medication administration
• Ability to work independently and as part of a team
• Compassionate patient-centered care approach
