Roles & Responsibilities

• Deliver comprehensive nursing care through the nursing process, maintaining the highest standards of professional nursing practice.
• Supervise and coordinate care with CNAs and other healthcare professionals to ensure seamless patient care.
• Perform comprehensive initial and ongoing assessments of patients' physical, psychosocial, environmental, and community resources.
• Develop, evaluate, and modify individualized plans of care in collaboration with patients, physicians, and caregivers.
• Establish discharge plans and coordinate with the care team to ensure smooth transitions.
• Complete OASIS data accurately to support patient care and regulatory compliance.
• Provide patient education and teaching strategies tailored to patients, families, and caregivers.
• Ensure thorough documentation in accordance with federal, state, and agency regulations.
• Promote a clean, safe environment of care using SAFE error prevention habits.
• Uphold the highest standards of privacy and confidentiality, adhering to the Standards of Conduct.

Qualifications

• Active Illinois RN license (or willingness to obtain)
• Home Health experience required; strong Med Surg experience with some Home Health will be considered
• Current BLS certification (or willingness to obtain)
• Valid driver's license and auto insurance
• OASIS and Start of Care experience highly preferred
• Must be fully vaccinated for COVID and Flu (declinations not accepted)
• Two supervisory references required

Knowledge & Skills

• Expertise in the nursing process and patient assessment
• Experience with care plan development and discharge planning
• Strong patient education and documentation abilities
• Supervisory skills for CNAs and care coordination
• Familiarity with OASIS and Start of Care for home health
• Knowledge of Med Surg nursing practices
• Current BLS certification
