Roles & Responsibilities

• Prescribe, coordinate, and deliver care through the nursing process
• Maintain the standards of professional nursing and supervise CNAs
• Provide comprehensive initial assessments and ongoing evaluations of physical, psychosocial, environmental, and community needs
• Initiate and modify the Plan of Care as ordered, and establish discharge plans with the patient, physician, and caregivers
• Complete OASIS data and documentation according to regulatory guidelines
• Educate patients, families, and caregivers to promote health and independence

Qualifications

• Active Registered Nurse (RN) license in Illinois
• BLS certification (or willingness to obtain)
• Home Health experience required
• Valid driver's license and car insurance
• COVID and Flu vaccination required (declinations not approved)
• OASIS and start of care experience strongly preferred
