Roles & Responsibilities

• Provide exceptional physical therapy services to residents in a skilled nursing facility
• Develop and implement individualized treatment plans to improve mobility and function
• Collaborate with nurses, physicians, and caregivers to ensure comprehensive patient care
• Document patient progress accurately and adjust therapies as needed
• Educate patients and families on therapeutic exercises and fall prevention strategies

Qualifications

• Active Physical Therapist (PT) license in California, or ability to obtain prior to start
• Doctor of Physical Therapy (DPT) degree
• New graduates welcome to apply - we provide mentorship and support
• Experience in skilled nursing facility (SNF) preferred but not required
• Strong clinical reasoning and communication skills

Education

• Doctor of Physical Therapy (DPT) degree

Knowledge & Skills

• Knowledge of physical therapy assessment and treatment techniques
• Ability to work with geriatric population and complex medical conditions
• Familiarity with SNF documentation standards (e.g., MDS, RAI)
• Excellent teamwork and interpersonal skills
• Proficiency in electronic medical records (EMR)
