Roles & Responsibilities

• Provide physical therapy assessments and treatments to patients in a skilled nursing facility
• Develop individualized care plans to improve mobility and function
• Collaborate with the interdisciplinary team for holistic patient care
• Document patient progress and adjust plans as needed
• Educate patients and families on exercises and home management strategies

Qualifications

• Active Physical Therapist license in Massachusetts
• BLS certification required
• Experience in skilled nursing or rehabilitation setting preferred
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team

Knowledge & Skills

• Knowledge of physical therapy modalities and techniques
• Patient assessment and goal setting
• Care plan development and documentation
• Adaptability to different patient needs
• Time management and prioritization
