Roles & Responsibilities
- Provide compassionate physical therapy services across four rural counties
- Conduct comprehensive Oasis Assessments for accurate patient evaluations
- Manage Start of Care cases with personalized treatment planning
- Deliver 4-6 meaningful patient visits daily using tablet-based tracking
- Help elderly patients recover from falls and fractures with expert care
Qualifications
- Current Physical Therapist license in Indiana
- Experience with Oasis Assessments and Start of Care documentation
- Comfortable working in rural community settings
- Strong organizational and time management skills
