Roles & Responsibilities
- Deliver personalized physical therapy services in patients' homes across Yolo County
- Assess and evaluate each patient's functional status to create tailored care plans
- Guide patients and families through therapeutic exercises and equipment use
- Monitor progress and communicate updates with physicians and care teams
- Coordinate with home health aides to support comprehensive rehabilitation goals
- Document clinical findings and celebrate patient milestones along the way
Qualifications
- Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
- Current BLS certification required
- Strong communication and organizational skills
- Valid driver's license and reliable transportation for home visits
Knowledge & Skills
- Home health physical therapy assessment and treatment techniques
- Patient and caregiver education strategies
- Understanding of Medicare and Medi-Cal regulations
- Ability to work independently while collaborating with interdisciplinary teams
