Candidate Pay Rate: $59 per hour

Roles & Responsibilities
- Deliver personalized physical therapy services in patients' homes
- Develop individualized treatment plans to help patients regain mobility and independence
- Collaborate with interdisciplinary care teams to ensure comprehensive patient support
- Educate patients and families on exercises and recovery techniques
- Document progress and adjust care plans to achieve optimal outcomes
Qualifications
- Active California Physical Therapist license
- BLS certification
- Home health experience preferred
- Strong communication and interpersonal skills
