Roles & Responsibilities

• Conduct thorough evaluations of residents to identify their occupational therapy needs.
• Develop and implement individualized treatment plans to help patients achieve their highest level of independence.
• Provide daily therapeutic treatments and interventions to promote rehabilitation and successful discharge to home.
• Collaborate with the interdisciplinary care team to ensure comprehensive and coordinated patient care.
• Participate in discharge planning and community reintegration to help residents transition smoothly.
• Maintain accurate and timely documentation in compliance with facility and regulatory standards.

Qualifications

• Valid Occupational Therapist license in the state of New Mexico or ability to obtain one.
• NBCOT certification as an Occupational Therapist (OT).
• 1-3 years of post-graduate experience, with SNF experience strongly preferred.
• Current CPR/BLS certification.
• Ability to meet productivity standards while delivering high-quality care.
• Strong communication and interpersonal skills.

• Knowledge of skilled nursing facility (SNF) regulations and documentation
• Experience with geriatric and rehab-to-home populations
