Roles & Responsibilities

• Provide compassionate, patient-centered care to residents in a skilled nursing setting
• Administer medications and treatments as ordered, accurately and safely
• Monitor and document vital signs, symptoms, and changes in condition
• Collaborate with RNs, physicians, and the care team to develop and implement care plans
• Assist with activities of daily living and ensure resident comfort and dignity
• Communicate effectively with residents, families, and team members
• Maintain a clean, safe, and organized environment following infection control protocols

Qualifications

• Active LPN license in Pennsylvania (or compact license where applicable)
• Current BLS/CPR certification (AHA or equivalent)
• Previous long-term care, skilled nursing, or geriatric experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to stand, walk, and lift/move patients as required
• A genuine passion for making a difference in the lives of others
