Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents in a long-term care or skilled nursing setting
• Administer medications, treatments, and injections as prescribed, with meticulous documentation
• Monitor and assess patient vital signs, symptoms, and overall condition, reporting changes promptly
• Collaborate with registered nurses, physicians, and interdisciplinary teams to develop and implement individualized care plans
• Educate patients and families on health conditions, medication regimens, and self-care strategies
• Maintain a safe, clean, and welcoming environment that promotes dignity and well-being

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico or a compact state
• Current BLS certification required
• COVID-19 vaccination or documented medical/religious exemption
• Previous experience in long-term care, skilled nursing, or rehabilitation preferred
• Ability to pass background checks and facility credentialing requirements

Knowledge & Skills

• Strong clinical assessment and critical thinking abilities
• Excellent communication and interpersonal skills for patient and team interactions
• Proficient with electronic health records (EHR) and medication administration
• Capable of working 12-hour shifts, including weekends, in a fast-paced environment
• A compassionate, patient-centered approach to care
