Roles & Responsibilities

• Provide comprehensive nursing care to patients in their homes, ensuring comfort and recovery.
• Conduct OASIS assessments to evaluate patient needs and develop individualized care plans.
• Coordinate care with physicians, therapists, and family members to optimize patient outcomes.
• Educate patients and families on disease management, medications, and home safety.
• Participate in on-call rotation for after-hours, weekend, and holiday coverage as needed.

Qualifications

• Active Registered Nurse (RN) license in Iowa
• Minimum 2-3 years of home health experience with OASIS assessments
• BLS certification
• Experience with electronic health records, especially Epic, is a plus

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Knowledge of home health regulations and OASIS documentation
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Proficiency in EHR systems
