Roles & Responsibilities

• Provide skilled physical therapy services to residents in a skilled nursing facility
• Develop and implement individualized treatment plans to improve mobility and function
• Collaborate with interdisciplinary team to optimize patient outcomes
• Document patient progress and adjust interventions as needed
• Float between Elk River Health & Nursing Center and its sister location in Ardmore

Qualifications

• Active Physical Therapist license in Tennessee (or compact license eligible)
• New graduates welcome to apply - we support your growth!
• Current BLS certification required
• SNF or geriatric experience preferred, but not required

Knowledge & Skills

• Strong clinical reasoning and assessment abilities
• Excellent communication and teamwork skills
• Ability to adapt to different patient populations and settings
• Proficiency with electronic documentation systems
