Roles & Responsibilities

• Evaluate and treat skilled nursing facility patients using evidence-based SLP approaches
• Create individualized treatment plans and set meaningful, measurable goals
• Complete timely, thorough clinical documentation in Net Health per facility standards
• Collaborate with the interdisciplinary care team to coordinate patient-centered care
• Provide flexible coverage and float to a nearby sister facility as required
• Engage in mentorship and professional learning opportunities to strengthen clinical skills

Qualifications

• Active California SLP license (required)
• Skilled Nursing Facility (SNF) experience preferred; mentorship available if experience is limited
• Strong documentation skills and familiarity with Net Health is a plus
• Excellent time-management and ability to prioritize caseloads
• Collaborative mindset and ability to work well with interdisciplinary teams
• Willingness to float to a sister facility within walking distance

Knowledge & Skills

• Proficient in patient evaluation and treatment planning for geriatric/SNF populations
• Clinical documentation best practices and experience with Net Health
• Interdisciplinary team communication and care coordination
• Time management, caseload organization, and efficient workflow
• Skilled in evidence-based SLP interventions commonly used in SNF settings
