Roles & Responsibilities
- Assess and evaluate patients’ therapeutic status and develop individualized care plans
- Direct home‑based physical therapy treatments and monitor progress
- Educate patients, families, and caregivers on therapeutic appliances and home exercises
- Collaborate with physicians and interdisciplinary teams to report treatment outcomes
- Supervise home health aides and coordinate additional home health services
- Maintain accurate clinical documentation and comply with Medicare and OSHA regulations
Qualifications
- Minimum of 2 years experience as a PT in acute, home care, or rehab settings
- BLS certification required
- Strong communication and organizational skills
- Ability to work independently in a home health environment
- Valid California PT license
