Roles & Responsibilities

Conduct physical therapy assessments and treatments for patients
Develop and implement individualized treatment plans
Collaborate with nursing staff and other healthcare professionals
Perform SOC visits, especially for orthopedic patients
Utilize EMR system for efficient documentation

Qualifications

2 years of experience in Physical Therapy, preferably in Home Health
BLS certification required
EMR experience, specifically with HCHB - HomeCare HomeBase
Physical Therapy licensure

Education

Physical Therapy degree from an accredited program

Knowledge & Skills

Strong clinical skills in Physical Therapy
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Familiarity with orthopedic and geriatric patient populations
