Roles & Responsibilities

Conduct physical therapy assessments and treatments for patients
Develop and implement individualized therapy plans
Collaborate with nursing staff and other healthcare professionals
Perform SOC visits as needed

Qualifications

2 years of experience as a Physical Therapist (will consider travelers with less experience and Home Health experience)
BLS certification required
Experience with Home Health and HCHB - HomeCare HomeBase EMR

Education

Physical Therapy degree from an accredited program

Knowledge & Skills

Strong clinical skills in physical therapy
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Familiarity with EMR systems
