Candidate Pay Rate: $91.06 - $63.74 per hour

Roles & Responsibilities
- Assess home care needs of frail elderly using the nursing process and develop initial plan of care
- Complete and submit Level of Care (LOC) determination forms for review
- Review medical records to complete LOC, using alternative info if records unavailable
- Conduct initial home/RN assessments at participants' homes during team in-home assessments
- Maintain an up-to-date active list of all LOCs submitted, including dates and decisions
- Follow up on deferred prospective enrollees and resubmit required info to DHCS for approval
- Participate in weekly enrollment team meetings and IDT meetings as needed
- Assist transition of care and monitor hospitalizations and SNF patients for appropriate transitions
- Communicate and interact with patient caregivers and family members as needed
Qualifications
- At least 1 year experience as an RN in home or community health with frail or elderly population
- Current license to practice as a Registered Nurse in California
- Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification (American Heart Association)
- High school diploma or GED equivalent
