Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a skilled nursing and rehabilitation setting
• Administer medications, treatments, and interventions accurately and safely in accordance with facility protocols
• Monitor and document patient vital signs, symptoms, and responses to treatment in a timely manner
• Collaborate with registered nurses, physicians, and interdisciplinary team members to develop and implement individualized care plans
• Communicate effectively with patients and families, providing education and emotional support
• Maintain a clean, safe, and organized environment, adhering to infection control and safety standards

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania
• Graduate of an accredited practical nursing program
• Current BLS/CPR certification (American Heart Association preferred)
• Previous LPN experience in long-term care, geriatrics, or skilled nursing is a plus
• Strong assessment, critical thinking, and clinical documentation skills
• Excellent communication and interpersonal abilities, with a patient-first attitude

Knowledge & Skills

• Solid understanding of nursing principles, pharmacology, and medication administration
• Proficiency in vital signs monitoring and patient assessments
• Comfort with electronic health records (EHR) and clinical documentation systems
• Ability to prioritize tasks and respond calmly in fast-paced environments
• Compassionate, empathetic approach to patient and family interactions
• Team collaboration and adaptability to changing patient needs
