Roles & Responsibilities
- Conduct physical therapy assessments and interventions for patients in home health settings
- Develop and implement individualized treatment plans
- Collaborate with nursing staff and other healthcare professionals
- Perform SOC visits, especially for orthopedic patients
Qualifications
- 2 years of experience in Physical Therapy, with Home Health experience preferred
- BLS certification required
- Physical Therapy licensure in California
Education
- Physical Therapy degree from an accredited program
Knowledge & Skills
- Physical Therapy evaluation and treatment techniques
- Home Health care principles and practices
- EMR systems, specifically HCHB - HomeCare HomeBase
- Strong communication and interpersonal skills
