Roles & Responsibilities

• Deliver compassionate physical therapy services to patients in a home health setting
• Develop and implement individualized treatment plans to help patients achieve their goals
• Document patient progress accurately in EPIC
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care
• Educate patients and families on therapeutic exercises and home safety
• Complete required competency testing and compliance checklists

Qualifications

• Active Physical Therapist license in Michigan
• Current BLS certification
• Experience with EPIC electronic health records
• Willingness to complete Trinity skills checklist and specialty specific competency testing
• Previous physical therapy experience preferred

Knowledge & Skills

• Strong knowledge of physical therapy assessment and treatment techniques
• Familiarity with EPIC documentation
• Excellent communication and patient education skills
• Ability to work independently and as part of an interdisciplinary team
• Time management and organizational skills
• Compassionate, patient-centered approach
