Speech Language Pathologist - Skilled Nursing Facility
Location: Farmington, New Mexico

Roles & Responsibilities
- Provide comprehensive speech and language therapy to residents in a skilled nursing setting
- Evaluate and treat communication, cognitive, and swallowing disorders
- Develop and implement individualized treatment plans to maximize functional outcomes
- Collaborate with the interdisciplinary team to ensure holistic resident care
- Document patient progress and maintain accurate clinical records
Qualifications
- Active Speech Language Pathologist license in New Mexico
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)
- Current BLS/CPR certification
- Minimum 1-3 years of clinical experience, SNF experience preferred
- Ability to float between San Juan Care Center and Farmington Wellness and Rehab
Knowledge & Skills
- Strong assessment and diagnostic skills in speech and swallowing therapy
- Excellent communication and interpersonal abilities
- Knowledge of geriatric therapy and skilled nursing facility regulations
- Ability to work independently and manage a varied caseload
- Flexibility to adapt to different facility environments
