Roles & Responsibilities
- Assist in crafting and executing personalized rehabilitation care plans with an interdisciplinary team
- Perform comprehensive head-to-toe assessments to track patient progress
- Administer IV medications, blood products, lab draws and breathing treatments with precision
- Educate patients and families on care plans to empower their recovery journey
- Utilize med/surg skills in a dynamic post-acute setting to make real differences
Qualifications
- Active RN license
- Minimum 1 year of current acute care Rehab RN experience
- BLS certification required
- Empathetic communicator who loves patient advocacy and education
Education
- RN licensure with acute care rehabilitation experience
Knowledge & Skills
- Meditech EMR experience a big plus
- CRRN Certification highly preferred
- Knowledge of Quality Indicator scoring in post-acute settings
- Comfort with patient lifts, iSTAT, glucometers and respiratory treatments
