Contract Type: Travel Contract

Shift Type: 5 days per week, 8-hour shifts; options: Monday-Friday or Tuesday-Saturday

Hours per Week: 40

Candidate Pay Rate: $77 per hour

Roles & Responsibilities

Provide skilled nursing care to home health patients with conditions such as CHF, COPD, wounds, and IV therapy
Perform wound care including wound vacs and manage PICC lines
Conduct start of care, recertification, resumption of care, and routine visits
Coordinate care with interdisciplinary team and communicate with patients and families
Document patient care accurately in HomeCare HomeBase (HCHB)

Qualifications

Current BLS certification
Valid driver's license and auto insurance
HomeCare HomeBase (HCHB) experience preferred
Minimum 1 year of home health experience preferred

Knowledge & Skills

Strong assessment and clinical decision-making skills
Ability to manage complex patient populations
Excellent communication and documentation skills
Knowledge of home health regulations and OASIS documentation
