Registered Nurse - Care Coordination
Location: Sacramento, California
Start Date: 07-07-2026
End Date: 10-06-2026
Contract Type: Travel Contract
Shift Type: 8-hour shifts, Monday-Friday, with flexibility for 12-hour night shifts for ED background candidates. Minimal time off requests preferred.
Hours per Week: 40
Candidate Pay Rate: $82 per hour

Roles & Responsibilities
- Conduct comprehensive discharge planning and utilization review
- Coordinate patient care and services
- Collaborate with healthcare teams to ensure smooth transitions
- Utilize software systems such as Midas, InterCall, Cerner, Curaspan, and Microsoft Office
Qualifications
- Registered Nurse (RN) license
- Minimum 3 years of hospital experience
- Acute level care coordination experience
- Proficiency in Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs
- Experience with discharge planning and utilization review
Knowledge & Skills
- Strong communication and interpersonal skills
- Ability to work in a fast-paced environment
- Excellent problem-solving and critical thinking skills
- Familiarity with Medicare/Medi-Cal guidelines
- Experience working with placement agencies
