Location: Woodland, California

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with rotating Saturdays once a month

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Assess, evaluate, and update therapeutic/rehabilitative/functional status of patients
Develop and implement physical therapy treatment plans in accordance with physician orders
Instruct patients and families in use of therapeutic appliances
Coordinate care with interdisciplinary team
Document progress and maintain clinical summaries
Supervise home health aides as needed

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab setting
Current BLS certification
Ability to provide 2 supervisory references from recent assignments
Must not have been directly employed by CommonSpirit, CHI, or Dignity within the past year

Knowledge & Skills

Physical therapy assessment and treatment
Home health and acute care experience
Strong communication and organizational skills
Knowledge of Medicare and Title 22 regulations
Ability to work independently
