Location: Santa Maria, CA

Contract Type: Contract
Shift Type: Flexible shifts, majority Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Provide skilled physical therapy services to patients in their homes
- Develop and implement individualized treatment plans
- Document patient progress using Oasis and other required systems
- Collaborate with interdisciplinary team for coordinated care
- Educate patients and families on home exercise programs
- Maintain compliance with all regulatory and company standards
Qualifications
- Current California Physical Therapy license
- BLS certification required
- Minimum one year of experience as a PT
- Home health experience preferred
- Experience with Oasis documentation required
Knowledge & Skills
- Knowledge of Oasis documentation system
- Orthopedic, stroke, joint, and spinal injury rehabilitation
- Home health setting processes and best practices
- Strong communication and organizational skills
