Roles & Responsibilities
- Conduct physical therapy assessments and interventions for patients in home health settings
- Develop and implement treatment plans for patients with various conditions, including orthopedic and geriatric patients
- Collaborate with nursing staff and other healthcare professionals to ensure comprehensive care
- Perform SOC visits, especially for orthopedic patients when nursing is not ordered
Qualifications
- 2 years of experience in Physical Therapy, preferably in Home Health
- BLS certification required
- Experience with EMR systems, specifically HCHB - HomeCare HomeBase
Education
- Physical Therapy degree

- Physical Therapy skills
- Home Health experience
- EMR system proficiency
- Communication and interpersonal skills
