Roles & Responsibilities

Conduct physical therapy assessments and interventions for patients in home health settings
Develop and implement individualized treatment plans for orthopedic and geriatric patients
Collaborate with nursing staff and other healthcare professionals to ensure comprehensive care
Perform SOC visits as needed, especially for orthopedic patients

Qualifications

2 years of experience in Physical Therapy, with Home Health experience preferred
BLS certification required
Physical Therapy licensure in California

Education

Physical Therapy degree from an accredited program

Knowledge & Skills

Physical Therapy assessment and intervention techniques
Home health care principles and practices
EMR systems (HCHB - HomeCare HomeBase)
Communication and interpersonal skills for working with patients and healthcare teams
